IVF

In vitro fertilisation (IVF) is now a commonplace term, familiar even to those people who have no need for such a procedure. It literally means that fertilisation takes place outside of the body; sperm are added to the ovum under laboratory conditions where fertilisation takes place. IVF is used as a treatment when a woman has anatomical changes in her fallopian tubes or has a gynaecological condition such as severe endometriosis. It is also recommended if other infertility methods have not produced the desired result within a year of trying, or in cases where there are mild pathologies in the sperm.

ICSI

In traditional IVF, the ovum is placed with thousands of swimming sperm under laboratory conditions. However, sometimes the sperm cannot penetrate the outer layer of the ovum. This may be because the ovum’s outer layer is too hard to penetrate or that the sperm are unable to swim. First performed in Belgium in 1992, intracytoplasmic sperm injection (ICSI) is an IVF procedure in which a single sperm is injected into the cytoplasm of the ovum.

ICSI is used in cases where a man’s sperm count is very low or the sperm are immobile and defective. Sperm of satisfactory quality are selected for the procedure and the best quality sperm is implanted inside the ovum using micro-instruments. ICSI is also performed in cases where previous IVF cycles have been unsuccessful.

Micro-TESE

Until recently, the standard treatment for azoospermia (no sperm at all in a man’s semen) was a procedure known as testicular sperm aspiration (TESA). During TESA, sperm is taken out of the testicle by means of a biopsy and then used for fertility treatment. The technique is effective in 30-50% of cases, a percentage that may be increased using TESE (testicular sperm extraction). However, TESA/TESE can be traumatic and induce extensive scarring in the testicle, decrease testosterone levels and further worsen sperm production.

Micro-dissection testicular sperm extraction (Micro-TESE) is a microsurgical procedure to remove sperm directly from the testicle. The procedure has a very low complication risk because the use of a high magnification microscope ensures that any tissue removed is pinpointed, increasing the probability of finding sperm cells and minimising the damage to surrounding tissues. Overall, Micro-TESE is twice as effective compared with TESA and has fewer side-effects.
